Some simple maths would sort out the NHS pronto

What would you do if you were the PM? All that power and all those problems.

A long time ago I opined on the idea that with great power comes great responsibility. Part of that responsibility is that we as commissioners will also be expected to deliver as providers. Part of that delivery is to make good on the shift of money from secondary to primary care. I reckon that £10 spent in primary care is the equivalent of £100 spent in secondary care. Read more »

Will you be striking for the NHS?

Are you striking? I don’t mean good looking, I mean exercising your right to withdraw your labour.

Of course, many of us are choosing to carry on working, struggling on with ‘mustn’t grumble’ as our refrain.

Although those selfless workers have dedication which is admirable, is it also misplaced. Other colleagues are supporting the action by craftily having a stealth strike. Read more »

What happens when you say no to the NHS wise men?

It is a very serious question that’s been taxing my brain this week.

A friend of mine is a clinical commissioning group (CCG) chair in a different region (yes truly, it is a friend, this isn’t one of those ‘condoms for a friend’ issues). Anyhow, he’s been very brave/stupid, by saying no.

The new 111 number: offering everyone the chance to be ill

Don’t tell me your heart didn’t leap with joy on hearing that we’re going to get the new 111 number off the ground. In the Black Tower we’ve been wrestling with how the local implementation might work for a while now.

Time to escape the collapsing Black Tower

I want a new job. The Black Tower is collapsing, the Cluster is goosed, the GPCC/CCGs are about to be herded together, demoralised and punished for all the evils of the system, but it’s okay – I know the job I want.

I’d like to be able to travel the country, meeting people to make a documentary on the evils of the NHS.

Spooky presence at the NHS Confederation conference

I enjoy the odd jolly about. I like the travel, usually first class, I like the catching up with old friends, the odd drinky poos in the hotel bar. In short I enjoy a good conference. This week I went ‘oop north’ to the NHS Confederation in Manchester.

Granted it was cold, wet, and took place in an old railway station, but this conference was remarkable.

Regime change has left the Black Tower eerily quiet

It’s been a fascinating week at the Black Tower
while the schools are off.

The few remaining skeletal staff are huddling around the
photocopier to keep warm. Senior colleagues are getting news that they have
been given interview dates for jobs that are not real jobs, in organisations
that are not real organisations.

How much do GPs know about the NHS reforms?

This week I was pretending to be a proper GP and attended a
meeting about finances. It was fascinating to meet other regular GPs from
beyond the realm of the Black
Tower.

Wait and GMC

I’ve been reading this week. It’s a bit of a surprise given
how much else there is to do in the rapidly shrinking Black Tower
- but those nice people at GMC are looking for comments. They’ve released a
consultation draft document entitled Good Management Practice: Guidance
for all doctors.
I’m afraid this is a must-read. It’s not long but each page is weighted down by
words that will trap the unwary and punish anyone who tries to take the short
cut through employment law and governance.
