Why general practice must change to survive

Allow me a brief moment of serious comment. GP needs to change. Not the august journal for which I blog, but the actual General Practice we all know and love. How do I know this fact? Read more »

No downside to the Health Bill!

I was speaking to a lawyer friend of mine. He has a passing interest in the NHS, having built his house on the spoils of litigation. Of course he’s been watching the progress or the Bill with great interest. His take on the outcome of it all is interesting. Read more »

Why are GPs responsible for everything?

Who elected the government? Who pays the bankers? Who decides when you will retire? Who decides which drugs you prescribe? Who decides which hours you’re open?  Who decides how much money it costs to run the practice? Who decides if you’re any good as a GP? Who decides which operations are “low priority”? Who closes the local AED? Who needs to spend more on British Pharmaceuticals to save UK PLC? Who commissions for Quality in General Practice? Who writes prescriptions for food supplements? Who stops patients from getting DLA? Who is responsible for all those benefits scroungers? Who supplies addicts with their gear? Who protects nurses from making decisions? Who runs the CCG? Can you feel a revolution in the air?

I know this may sound odd coming from an ex-PCT medical director but I’m fortunate enough to count our local LMC chairs amongst my circle of friends.

What happens when you say no to the NHS wise men?

It is a very serious question that’s been taxing my brain this week.

A friend of mine is a clinical commissioning group (CCG) chair in a different region (yes truly, it is a friend, this isn’t one of those ‘condoms for a friend’ issues). Anyhow, he’s been very brave/stupid, by saying no.

The seas of commissioning

I met with our GPCC/CCG/CCC chairs this week. They are both very smart, experienced GPs who have steered their crafts through uncharted waters. Apparently the map they should be using is due out ‘at the end of the month’. The authorisation process will explain to our intrepid explorers whether the vessel in which they have commenced the journey is to be allowed to progress further.

Can the new GP leaders win the trust of primary care?

A key part of being a leader is to be trusted by those we lead. Many of us think that trust is something which has to be earned, takes time and either happens or not.

At the Black Tower we’ve had some cracking leaders over the last decade, trouble is they never stick around for more than a year or two before being whisked off to bigger and better pastures.

Regime change has left the Black Tower eerily quiet

It’s been a fascinating week at the Black Tower
while the schools are off.

The few remaining skeletal staff are huddling around the
photocopier to keep warm. Senior colleagues are getting news that they have
been given interview dates for jobs that are not real jobs, in organisations
that are not real organisations.

Working at the PCT is like being at the Somme in 1915

I think it’s fair to say that up close the Silver Fox is a reasonably attractive individual.

He’s not my personal cup of tea, you understand, but ageing well, full head of grey hair and a reasonably charismatic style that all senior politicians seem to develop.

How much do GPs know about the NHS reforms?

This week I was pretending to be a proper GP and attended a
meeting about finances. It was fascinating to meet other regular GPs from
beyond the realm of the Black
Tower.
